Facility Evaluation Report
On February 12, 2026, Licensing Program Analyst (LPA) Kayla Adkison arrived at the facility unannounced for a case management visit. LPA met with Jennifer Boss, Administrator and Natasha Peerman, assistant administrator and explained the purpose of the visit. During the visit, there were four (4) staff providing care and 19 residents in the facility.
LPA made visual observations of the facility and interviewed Administrator and Assistant Administrator regarding an incident that occurred in April 2024.
No deficiencies are being cited as a result of this visit. Exit interview conducted. A copy of this report was provided, via email, to Jennifer Boss, Administrator.
