The inspector’s account

On June 30, 2026, Licensing Program Analyst (LPA), Ivan Avila arrived at the facility unannounced to conduct a case management visit. LPA met with Administrator Robert Holmes and explained the purpose of the visit. The purpose of today's visit is to follow up on an Unusual Incident/Injury Report that was sent to the Department on June 24, 2026.

During the incident on June 24, 2026, staff stated R1 ran out of their room screaming and yelling out that their chest hurt. Staff noticed blood on R1's face and immediately called emergency services. R1 ran into the bathroom when staff contacted 911. While staff was on the phone with 911, R1 collapsed in the bathroom and staff immediately implemented CPR following the direction of the dispatcher until EMTs arrived. EMTs arrived approximately eight to ten minutes later and took over CPR procedures.

During today's follow up visit, LPA collected documents for the resident in question.LPA informed Administrator at this time, the current matter is being under review by the Department.

No deficiencies were cited during today's Case Management - Incident Inspection. Exit interview was conducted and a copy of this report was signed and given to the Administrator.
