On 10/1/21 at 10:30 AM, Licensing Program Analyst (LPA) Pheej Cheng and Dawn Keane conducted an unannounced case management visit as directed by the department and met with Interim Administrator Cameron Olson. Prior to initiating the visit, LPA completed required COVID-19 testing protocols, and a daily self-screening questionnaire for symptoms of COVID-19 infection to affirm no COVID-19 related symptoms; contacted Administrator and completed a facility risk assessment. LPA ensured they applied hand sanitizer before entering the facility and the following Personal Protective Equipment (PPE) was worn: surgical mask and gloves. Additionally, LPA was screened by staff Olson.

A case management visit was held on 10/1/21 to follow up on a substantiated allegation of physical/sexual abuse. Licensing Program Analyst (LPA) Pheej Cheng and Dawn Keane met with Interim Administrator, Cameron Olson, at Joaquin House in Redding, California.

On April 8, 2021, the Department concluded a complaint investigation which alleged that the staff physically/sexually abused a resident in care.

The Department substantiated the incident’s investigation, and the licensee was cited for violating California Code of Regulations (CCR) Title 22, § 80072(a)(2) Personal Rights. During the course of the investigation, it was determined that the facility did not protect Resident 1 (R1)’s dignity in personal relationship with staff and that this posed an immediate health and safety risk to clients in care.
