Facility Evaluation Report

On April 17, 2025, Licensing Program Analyst (LPA) Ivan Avila arrived at the facility unannounced to conduct a 1-year annual inspection. LPA observed the facility to be vacant and fenced off.

This facility suffered significant fire damage back in November 2024 to the outside and inside of the facility and there are no residents in care. The licensee wishes to maintain the license in the event the facility is rebuilt. There are no medications being stored and no indication that anyone resides in the facility.

No Deficiencies being cited as a result of today’s visit.
