The inspector’s account

On 12-9-25 Licensing Program Analyst LPA Sarah Benson arrived at the facility to conduct a case management visit. LPA met with Memory Care director Waniesha Ortiz and explained the reason for the visit.

The department is following up on an Incident Report (IR) submitted by the facility on 11-15-25 concerning a fall with an injury.

It was reported on 11-15-25 resident R1 had fall with injury. Care staff was conducting rounds when they found R1 on the floor between her bed and dresser. Staff conducted a full body check. R1 complained of pain in her head, back, and hips. R1 had a bump on the back of head and skin tear on right forearm. R1 stated she fell out of bed. EMS arrived and transported R1 to Hospital.

At this time, this case is under review and the department will do follow up as needed.

No citations were issued per Title 22 Regulations.

Exit interview conducted and copy of the report left at facility.
