On July 22, 2026, Licensing Program Analyst (LPA) Ivan Avila, conducted a case management visit regarding an incident that was reported to the Department. LPA met with House Manager Bobbie Mcdonald and explained the purpose of the visit.
The department is following up on an Incident Report that occurred on July 18, 2026 concerning a client.
LPA obtain further information regarding R1. During today's inspection LPA interviewed House Manager, R1, and staff. LPA collected additional documents.
No deficiencies cited during today's inspection. Exit interview conducted and copy of the report was provided.
