Facility Evaluation Report
On May 15 2026, Licensing Program Analyst (LPA) Sarah Benson arrived at the facility for the purpose of conducting the annual inspection. LPA met Manager Shirley Robichaux at the facility to verify no residents are living at the facility. The facility is owned by Licensee, Quality Care Home, INC. LPA observed and was informed there were no longer any residents at Hallmark North. LPA was informed the licensee intends to keep the license open at this time and possibly serve additional residents at a later date.
A copy of this report was provided to Manager Shirley Robichaux, to forward to Licensee.
An exit interview was conducted, and a copy of the report was left with the manager. No deficiencies are being cited as a result of todays visit.
