Facility Evaluation Report

On July 17, 2025, Licensing Program Analyst (LPA) Kayla Adkison arrived at the facility for the purpose of conducting the annual inspection. Upon arrival, LPA attempted to make contact with staff at the front door however, there was no answer. LPA was contacted by Lovene Ferrell, staff at Westside Assisted Living, located across the street at 915 Hallmark Drive, Redding. Both facilities are owned by Licensee , Quality Care Home, INC . LPA was informed there were no longer any residents at Hallmark North. The licensee intends to keep the license open at this time and possibly serve additional residents at a later date.
