The inspector’s account

On 08/19/2025 Licensing Program Analyst (LPA) Sarah Benson conducted an unannounced visit to the facility for closure and a final walk through. No one was present at LPA's arrival.

LPA Benson received notice from the Licensee 8-14-25 of closure. The licensee reported selling the home not as an RCFE but as a residential home.

The purpose of this visit was to determine whether there were any residents in care. The licensee reported the house has had no residents in care for over a year, no residents are living at Hallmark House.

LPA Benson rang the door bell, no one answered the door. LPA Benson observed no residents in care.

LPA requested the facility license to be returned to licensing.

No deficiencies were cited during this visit. A copy of the report was emailed to the licensee for a signature.

Licensee will return a copy of signed report to licensing.
