On July 24, 2025, Licensing Program Analyst (LPA) Kayla Adkison arrived at the facility to conduct a required 1-year inspection. Upon arrival, LPA made contact with MIchael Moodie, and was informed the facility currently has no residents in care. LPA visually confirmed the facility is not currently serving any residents. Facility Manager informed LPA that Licensee intends to sell the property and will inform licensing of confirmed sale for forfeiture of license. LPA observed a for sale sign in the front yard of the property.
Exit interview conducted and a copy of this report was provided to Michael Moodie, Facility Manager.
