On May 19, 2026, Licensing Program Analyst (LPA) Ivan Avila, conducted a case management visit. LPA met with facility representative, Administrator Amber Watkins and explained the purpose of the visit.
The department is following up on an Incident Report submitted by the facility on May 12, 2026, involving a resident.
LPA obtain further information regarding R1's hospitalization and surgical procedure.
No deficiencies cited during today's in inspection. Exit interview conducted and copy of report was provided.
