The inspector’s account

On 05/06/2026 Licensing Program Analyst (LPA) Marisa Chiarelli, arrived at the facility unannounced to conduct a 1-Year Required Annual Inspection, LPA Chiarelli met administrator Kiara Walker.

LPA Chiarelli and Administrator toured facility together to ensure health and safety of residents in care. Areas toured include but are not limited to: common areas, four (4) resident bedrooms, backyard, shed, and (2) two common restrooms. Administrator informed LPA Chiarelli that the storage shed will be getting replaced (refer to technical assistance for further notes).

LPA observed the residents bedrooms to have all the required furnishings, windows with screens and working lights.

Facility has a 2-day perishable and a 7-day non-perishable amount of food. LPA observed all toxins, medications, and sharps to be locked and inaccessible to residents. Hot water temperature was measured within the required range.

LPA observed one (2) fire extinguisher, fire detectors, and carbon monoxide detectors. Fire extinguisher was last serviced in August 2025. The facility is conducting emergency disaster drills monthly. First aid kit was fully stocked and ready for use.

LPA reviewed a total of four (4) residents' files and three (3) staff files which contained all of the required documentation. LPA reviewed two (2) residents medications.

Several topics were discussed. No deficiencies are being cited as a result of this inspection.

An exit interview was conducted and a copy of this report was provided to administrator.
