Facility Evaluation Report
On May 29, 2025, at 10:30 AM, Licensing Program Analyst (LPA) Kayla Adkison, arrived at the facility unannounced to conduct a 1-Year Required Annual Inspection. LPA met with Administrative Assistant, Crystal Monismith, and explained the purpose of the visit. Administrator Kiara Walker joined the meeting approximately 20 minutes later.
LPA Adkison and Administrative Assistant toured facility together to ensure health and safety of residents in care. Areas toured include but are not limited to: common areas, four (4) resident bedrooms, backyard, shed, and (2) two common restrooms.
LPA observed the residents bedrooms to have all the required furnishings, windows with screens and working lights.
Facility has a 2-day perishable and a 7-day non-perishable amount of food. LPA observed all toxins, medications, and sharps to be locked and inaccessible to residents. Hot water temperature was measured within the required range. LPA observed the dishwasher to be out of order. Staff is currently hand-washing all dishes and there was no evidence of any dirty dishes in the kitchen or common areas. LPA discussed with Administrator having the dishwasher repaired. A technical assistance as issued (See LIC 9102).
LPA observed one (1) fire extinguisher, fire detectors, and carbon monoxide detectors. Fire extinguisher was last serviced in August 2024. The facility is conducting emergency disaster drills monthly with the last being conducted in May 2025. First aid kit was fully stocked and ready for use.
LPA reviewed a total of four (4) residents' files and two (2) staff files which contained all of the required documentation.
Several topics were discussed. No deficiencies are being cited as a result of this inspection.
An exit interview was conducted and a copy of this report was provided to administrator.
