Facility Evaluation Report

On May 7, 2026, Licensing Program Analyst (LPA) Ivan Avila arrived at the facility unannounced to conduct a case management visit and met with Makhila Hurst and explained the purpose of the visit.

LPA Avila explained that the department received an Exclusion Action: Decision And Order letter for an employee. LPA Avila informed Administrator Makhila Hurst the employee is prohibited from employment in, presence in, and contact with clients of, any facility licensed by the Department and must be removed. LPA informed Administrator that the letters were sent to both, the employee and Curtis Homes, LLC certified mail on February 20, 2026 . Administrator stated the employee is not working for the company and is not associated to the facility or facilities operated by Curtis Homes, LLC. LPA told Administrator that the Decision And Order was effective on March 2, 2026 .

LPA verified the employee is not present or working at the facility.
