Facility Evaluation Report
On 11/18/2025 Licensing Program Analyst (LPA) Sarah Benson conducted an visit to the facility for a closing final walk through. LPA Benson met with Licensee Yvette Montgomery for closure and a final walk through.
The purpose of this visit was to determine whether there were any residents in care. Licensee verified the last resident moved out on 11-14-25. LPA Benson and the licensee toured the facility together. The facility was clean and organized with no residents in care. The licensee reported moving the two residents to their other home.
No deficiencies were cited during this visit.
Exit interview was conducted and a copy of this report was signed and given to the Administrator.
