Facility Evaluation Report
On March 14, 2026, Licensing Program Analyst (LPA) Cassie Yang arrived unannounced at the facility to conduct a case management regarding facility's financial stability and current operation. LPA met with lead medication technician (Med Tech) and explained the purpose of the visit.
During today's visit, LPA and Med Tech conducted a tour of the facility. LPA observed ten residents present in the dining area, eating breakfast. LPA was informed the other three residents were in their bedrooms. LPA observed four staff working at the facility. LPA observed facility to have at least two days of perishable and seven days of nonperishable foods. LPA was informed that a big food order just recently arrived at the facility. Breakfast served during LPA's visit was biscuit and gravy with spam and eggs along with coffee, juice and tea. LPA observed residents to be clean and socializing in the common area.
It was discussed about payroll, possible relocation of residents in care, and facility closure in the future. Facility updates are to be provided to LPA Avila.
