Facility Evaluation Report

On Friday, February 27, 2026, an informal conference was conducted virtually via Microsoft Teams Meeting. Present in the meeting was Acting Regional Manager, Troy Ordonez, Licensing Program Analyst (LPA) Ivan Avila, and CEO, Kenneth Freel representing Golden Years LLC dba A Touch of Heaven

CEO was informed that this Informal conference is a part of the Administrative Action process and that further noncompliance may result in an elevation to a formal noncompliance conference, which could lead to a referral to the Department's legal division for possible revocation of license.

The purpose of today’s meeting was to discuss serious concerns regarding LLC’s financial stability and its ability to maintain continuous operations.
