Facility Evaluation Report
On 1-14-26 Licensing Program Analyst LPA Sarah Benson arrived at the facility unannounced to complete a case management in response to an incident report dated 1-14-26. . LPA met with administrator Sarah Puckett and explained the purpose of the visit.
The facility was toured, records were reviewed and the facility was toured. The incident report stated a client was able to obtain the medication lock box keys and administered medication.
Deficiencies cited from the California Code of Regulations, Title 22, Division 6 of California Regulation. Failure to correct the deficiency and/or repeat deficiencies within a 12-month period may result in civil penalties.
An exit interview was conducted, and a copy of the report and appeal rights were provided to the administrator.
