Facility Evaluation Report
Licensing Program Analyst (LPA) David Marrufo conducted an unannounced Case Management - Annual Continuation visit and met with Administrator Marie Kathleen Alcala.
During visit, LPA tested the water temperatures in the bathroom sinks in the front and rear buildings. The water temperatures ranged from 115 F to 118 F.
LPA reviewed the resident records for 6 residents, including Centrally Stored Medication and Destruction Records and Personal and Incidental Money Logs, and found all records to be complete.
LPA Marrufo requests the following documents be updated and copies sent to the department by 12/19/2025:
LIC308 Designation of Administrative Responsibility
LIC400 Affidavit Regarding Client/Resident Cash Resources
No deficiencies were cited at this time as per California Code of Regulations Title 22. This report was reviewed with Administrator Alcala and a copy of this report was provided.
