Facility Evaluation Report
Licensing Program Analyst (LPA) David Marrufo conducted an unannounced Case Management visit and met with Administrator (ADM) Marie Kathleen Alcala. The purpose of the visit was to respond to an incident involving the facility water pump being broken, which the facility reported via Unusual Injury/Incident Report (IR) on 05/20/2025.
The IR stated that on 05/16/2025 at approximately 6:30 PM, the water in the facility stopped flowing. A plumber was called and upon inspection, suspected that an electrical issue may have caused the water to stop flowing. An electrician confirmed that the water pump had burned out. Staff provided drinking water to residents and manually refilled toilets to maintain usability. Portable bathrooms and hand washing stations were brought to the facility by 05/17/2025. Staff transported residents to a sister facility to use their showers. On 05/20/2025, the facility water pump had been replaced and water flow had been restored.
During visit on 05/28/2025, LPA Marrufo toured the facility and observed three out of three bathrooms. Each bathroom had water flowing in the toilets, sinks, and showers. The water temperatures in the bathroom sinks ranged from 105 F to 115 F. LPA toured the outside area and observed the portable bathrooms and hand washing stations were still on site. ADM stated she will be keeping the portable bathrooms and hand washing stations on site until the end of the month as a precaution. LPA Marrufo observed the newly installed water pump.
No deficiencies were cited at this time as per California Code of Regulations Title 22. This report was reviewed with ADM Marie Kathleen Alcala and a copy of this report was provided.
