Facility Evaluation Report
Licensing Program Analyst (LPA) David Marrufo conducted an unannounced Required 1 - Year visit and met with Program Director Maria Mendoza.
During visit, LPA Marrufo toured the facility inside and out. LPA toured the facility activity rooms and the garage area. LPA toured the outside exits and found them to be clear of obstructions.
LPA toured five out of five resident bathrooms. Each bathroom had working lights and available soap and paper towels. The water temperature from the sinks in four out of five bathrooms ranged from 111 F to 113 F. One out of the five bathrooms had water temperature that measured at 81 F. This bathroom has been awaiting repairs since the previous LPA visit on 01/06/2026.
During visit, LPA tested two out of two carbon monoxide detectors and they functioned properly when tested. LPA observed that the smoke detection system has a most recent inspection date of 11/2025.
LPA Marrufo reviewed the medications for four clients who have medications centrally stored at the facility. Four out of the four client's records did not have a current Centrally Stored Medication and Destruction Record. LPA Marrufo reviewed 7 client records and found them to be complete. LPA reviewed 7 staff records and found them to be complete.
