Facility Evaluation Report
Licensing Program Analyst (LPA) David Marrufo conducted an unannounced Case Management - Legal/Non-Compliance visit and met with Program Director Maria Mendoza.
During visit, LPA Marrufo checked the water temperatures in four out of five resident bathrooms. Each observed bathroom sink had water temperatures ranging from 108F to 111 F. The bathroom in the facility gym was closed due to maintenance during visit.
LPA Marrufo advised Program Director Maria Mendoza to submit an Unusual Injury/Incident Report (IR) to the department to report that the gym bathroom is closed and undergoing maintenance. The IR report should report how the facility will ensure that clients will continue to be able to use the bathroom while the gym bathroom is closed and the facility's plan to repair the bathroom.

No deficiencies were cited at this time as per California Code of Regulations Title 22. This report was reviewed with Program Director Maria Mendoza and a copy of this report was provided.
