The inspector’s account

Licensing Program Analyst (LPA) David Marrufo conducted an unannounced Case Management Visit and met with Lacisha Zapata-Smith.

The purpose of the visit was to follow up on an Incident Report self-reported by the facility on 02/13/2026. The Incident Report stated that on 02/06/2026, Client C1 reported to Program Director Maria Mendoza that staff S1 yelled at C1, grabbed C1's arm, and told C1 not to tell anyone that S1 grabbed C1's arm.

During visit, LPA Marrufo conducted interviews with staff.

No deficiencies were cited at this time as per California Code of Regulations Title 22. This report was reviewed with Lacisha Zapata-Smith and a copy of this report was provided.
