The inspector’s account

Licensing Program Analyst (LPA) David Marrufo conducted an unannounced Case Management Visit and met with Nohemi Manzo Acuna.

The purpose of the visit was to follow up on an Incident Report self-reported by the facility on 02/13/2026. The Incident Report stated that on 02/06/2026, Client C1 reported to Program Director Maria Mendoza that staff S1 yelled at C1, grabbed C1's arm, and told C1 not to tell anyone that S1 grabbed C1's arm.

During visit, LPA Marrufo obtained copies of client and facility records. LPA Marrufo conducted a telephone interview with a witness.

No deficiencies were cited at this time as per California Code of Regulations Title 22. This report was reviewed with Nohemi Manzo Acuna and a copy of this report was provided.
