Facility Evaluation Report

Licensing Program Analyst (LPA) Marybeth Donovan conducted a Technical Assist (TA) Visit via Zoom platform with Jessica Greco Regional Director, Toni Rivera Program Clinical Consultant, and Jackie Jin Licensing Program Manager to provide technical assistance to prevent and mitigate the spread of COVID 19 in the facility. LPA conducted a virtual tour of the facility.

During today's TA-Visit, recommendations were made to as follows:

1. Post Mask Signage outside at the entrance to the facility.

2. Utilize foot operated trash cans in the bathrooms and kitchen area

3. Review OSHA Guidelines on N95 Fit Testing

Report reviewed with Jessica Greco Regional Director and a copy emailed for signature.
