Facility Evaluation Report

Licensing Program Analyst (LPA) David Marrufo conducted an unannounced Case Management visit and met with Krista Scally.

During visit, LPA Marrufo reviewed 12 out of 12 resident records and 4 staff records. The resident records of residents R1-R4 had incomplete Appraisal/Needs and Services Plans. R1-R2's records had completed Initial Service Plans and their move-in dates were 7/28/2025, and 7/15/2025 respectively. R3's Appraisal/Needs and Services Plan did not have a signature and signature date from the licensee and did not have a signature date from the resident. R4's Appraisal/Needs and Services Plan did not have signatures and signature dates from the licensee and resident.

Staff S1-S3 had completed training on developing and updating needs and services plans and had more than 20 hours of annual training. S4 began employment at the facility on 5/16/2025 and has 11.25 hours of training. S4's training on developing and updating needs and services plans was not available during visit.

No deficiencies were cited at this time as per California Code of Regulations Title 22.
