Facility Evaluation Report

Licensing Program Analyst (LPA) Marcella Tarin arrived unannounced to conduct a Plan of Correction (POC) visit. LPA met with Administrator (ADM) Jennifer Ryder. LPA stated the purpose of the visit.

On 6/6/2025 LPA conducted a Case Management visit for a Medication Error reported to the Department on 6/5/2025. The Incident Report (IR) stated Client C1 did not receive the correct medication dosage on 6/4/2025. A Type A deficiency was issued and a Plan of Correction (POC) was developed with the ADM. The POC was submitted by the POC due date.

LPA cleared the deficiency and provided ADM with a Letter of Deficiency Citations Cleared during visit.

No citations were issued during today's visit. An exit interview was conducted with ADM and a copy of this report was provided
