Facility Evaluation Report
Licensing Program Analyst (LPA) Marcella Tarin arrived unannounced to conduct a Case Management-Incident visit for an incident report regarding an elopement. LPA met with Administrator (ADM) Jennifer Ryder and stated the purpose of the visit. LPA observed 3 staff and 4 clients in the backyard gardening during visit.
On 4/1/2025 the Department received an incident report for resident R1, who eloped to the neighboring facility, Camphill Communities II at approximately 5:31 a.m. and returned back to Camphill Communities California at approximately 5:40 a.m. The facilities are approximately 100 yards from each other. R1 was unharmed during the elopement.
LPA toured the facility with ADM to include but not limited to residents bedrooms, staff bedrooms, parking lot, gated entrance and pathway alongside front of facility.
LPA interviewed 5 staff, 1 resident, and ADM. LPA requested the following documentation to include a copy of R1's needs and service plan, staffing roster for 4/1/2025, physician's report and video footage of incident.
LPA determined that this incident requires further investigation.
No citations were issued during today's visit per California Code of Regulations Title 22. An exit interview was conducted with Administrator Jennifer Ryder and a copy of the report was provided.
