Facility Evaluation Report
Licensing Program Analyst (LPA) Marcella Tarin arrived unannounced to conduct a Case Management-Incident visit regarding an Incident Report (IR) submitted to the Department on 6/18/2025. LPA met with Administrator (ADM) Monica Hernandez. LPA stated the purpose of the visit.
On 6/18/2025 the Department received an Incident Report regarding an incident involving Resident R1 and Resident R2 on 6/17/2025.
LPA interviewed ADM and 2 residents regarding the incident on 6/17/2025. ADM stated R1 and R2's treatment teams have been notified, and R1 and R2 are being monitored more frequently by staff.
LPA reviewed resident files, and pertinent documentation.
LPA determined this case management does not require additional investigation.
No deficiencies were cited during today's visit. An exit interview was conducted with ADM and a signed copy of this report was provided.
