Facility Evaluation Report

Licensing Program Analyst (LPA) Marcella Tarin arrived unannounced to conduct a Case Management-Other visit. LPA met with Licensee Andy Pereira and Administrator (ADM) Trevor Christiansen. LPA stated the purpose of the visit.

The purpose of this visit is to follow up on a "Decision and Order" for the exclusion of Staff S1. The Department of Social Services Community Care Licensing Division has issued a Decision and Order for the exclusion of S1, effective 09/25/2025. A copy of the Decision and Order was provided and discussed with Licensee and ADM.

Licensee and ADM state S1 is no longer working at the facility. ADM provided LPA with a copy of the staff schedule for 11/7/2025 and S1 is not on the staff schedule.

LPA advised Licensee and ADM that S1 is not allowed to work and volunteer in any licensed facilities.

No deficiencies cited today per California Code of Regulations, Title 22. An exit interview was conducted with ADM, and a copy of this report was provided
