Facility Evaluation Report
Licensing Program Analyst (LPA) David Marrufo conducted an unannounced Required 1 Year visit and met with Janet Leon.
LPA toured the facility bedrooms and observed each bedroom had working lights and available bedding and clothing storage areas. LPA tested the smoke and carbon monoxide detectors in the hallways and resident rooms and observed that they all functioned properly when tested.
LPA toured two out of two resident bathrooms and observed each bathroom had working lights and available soap and paper towels. The water temperatures in the bathroom sinks measured at 110 F and 111 F.
LPA toured the outside area and observed the outdoor exits to be clear of obstructions.LPA reviewed five out of five staff record and found them to be complete. The last fire drill was conducted on 01/16/2026.
