Facility Evaluation Report
On 6/15/2026, LPA Grace Donato conducted an unannounced Required 1 Year inspection and met with Tina & Dale Thornbrugh.
During visit, LPA toured the facility inside and out. LPA toured the kitchen area and observed there to be a perishable food supply of at least two days and a non-perishable food supply of at least seven days.LPA toured the exterior of the facility and observed the outdoor exits were free of obstructions. Swimming pool is fenced. LPA toured three resident bedrooms. Each bedroom had working lights and available bedding and clothing storage areas. LPA toured one out of one facility bathroom. The bathroom sink had water temperature at 106 F. The bathroom had working lights and available soap and paper towels.
Four resident records and three staff records were reviewed. Resident records are updated, complete and signed. Staff records are complete, with training logs. Staff have current First Aid/CPR certifications on file.
Centrally stored medication was locked and inaccessible by residents.
No deficiencies cited today. Report is reviewed and copy is provided.
