Facility Evaluation Report
*This is an amended report to correct the listed name of Administrator (ADM) Victor Cortes, which LPA inadvertently listed as Mark Cortes on the LIC809 dated 11/4/2025*
Licensing Program Analyst (LPA) Marcella Tarin arrived unannounced to conduct the facility's Required 1-Year annual inspection. LPA met with with Administrator (ADM) Victor Cortes and Associate Director (AD) Mark Smith. LPA stated the purpose of the visit. ADM and AD stated the facility had 18 clients and 11 staff.
LPA inspected the interior and exterior of the facility with ADM and AD to include but not limited to the kitchen, client activity area, client bathrooms, and outside patio and garden area. LPA measured 2 client bathroom sink water temperatures with a range from 107.7 F to 108.8 F. LPA observed the knives and chemicals locked and inaccessible to clients in care. LPA observed the refrigerator temperature at 30 F and Freezer at -10 F.
The facility was equipped with smoke and carbon monoxide detectors. All smoke detectors functioned properly when tested by ADM. Fire extinguishers were last serviced on 9/25/2025. LPA reviewed the facility first aid kit, and it was observed to be complete. The facility emergency drill log was reviewed. The facility's last drill was on 6/26/2025.
LPA reviewed 3 Client Records and 3 Staff records.
No deficiencies were cited per California Code of Regulations, Title 22. An exit interview was conducted with ADM and AD, and a copy of this report was provided.
