Facility Evaluation Report
Licensing Program Analyst (LPA) Ryker Heberle conducted an unannounced Required - 1 Year visit on 11/15/2021 at 10:25am and met with Administrator Victor Cortes (Admin).
During visit, LPA toured the facility. LPA toured the reception area, activity room, kitchen, 2 bathrooms, and storage closet. The facility currently has clients attending the program. LPA was screened for symptoms and temperature upon entrance into the facility at designated entry point. All staff members observed to be wearing masks, clients sitting at main activity table noted to not be wearing masks. LPA observed PPE supplies and COVID-19 related posters in the facility. LPA observed that the facility did not have any N95s or gowns.
No prohibited items noted in facility. All emergency exits noted to be clear of obstruction. All rooms in facility noted to be clean and well maintained. Hand sanitizers, soap, and paper supplies were observed to be available. At least 2 days' supply of perishable food and at least 1 week's supply of non-perishable food was observed on the premises.
All restrooms stocked with paper towels. Hand washing signs observed in all bathrooms. Facility water temperature measured at 109.5 *F Social distancing signs observed to be posted in all public areas. Staff has not completed N95 fit testing. Facility was not observed to have a designated room for residents to isolate in the event of manifestation of symptoms. Vaccination rate for staff and clients is currently at 100%.
No deficiencies cited during today's visit. Advisory notes issued. This report was reviewed with Administrator Victor Cortes and a copy of the signed report was provided.
