Facility Evaluation Report
Licensing Program Analyst (LPA) Marcella Tarin conducted an unannounced Case Management visit regarding an allegation of violation of personal rights that occurred on 12/2/2025. LPA met with Administrator (ADM) Alicia Olivarria. LPA stated the purpose of the visit. ADM states 3 residents were at day program, and 1 resident had just returned from day program.
On 12/4/2025 the Department received an incident report stating Staff S1 had pulled Resident R1's hair on 12/2/2025. The incident was reported by Staff S2 to S3, who reported to ADM on 12/3/2025, and ADM reported to Licensee on 12/3/2025. On 12/3/2025 the facility conducted an internal investigation, and were unable to substantiate the alleged hair pulling by S1. S2 was placed on leave on 12/3/2025, and was terminated on 12/5/2025.
LPA reviewed the Guardian website and observed S1 was no longer associated to the facility or any Next Step, Inc facility as of 12/5/2025.
During today's visit, LPA interviewed ADM. ADM states R1 is doing well and there have been no incidents of violation of personal rights. ADM states R1's responsible parties were notified.
LPA reviewed 1 staff file, and pertinent documentation.
LPA determined this case management does not require additional investigation.
No deficiencies were cited during today's visit. An exit interview was conducted with ADM and a signed copy of this report was provided.
