Facility Evaluation Report
On 5/7/2026, Licensing Program Analyst(LPA) John Calandra arrived at the facility to conduct a Case Management visit in regards to an Unusual Incident Report received by the Department on 5/5/2026. LPA Calandra was greeted by Cherie Mix, Business Office Coordinator and explained the purpose of the visit.
According to the Incident Report, R1 reported to the facility that approximately $100 had gone missing from their safe. According to Cherie, the Business Office Coordinator she conducted an internal investigation of the incident and found that it was an outside party and not facility staff who likely committed the act. Cherie helped R1 file a police report which LPA Calandra was able to obtain a copy of during the visit. In addition, LPA Calandra received copies of the staff interviewed by Cherie.
No deficiencies cited during today's visit.
An exit interview was conducted and a copy of the report provided to facility representative.
