Facility Evaluation Report
On 6/17/2026, Licensing Program Analyst(LPA) John Calandra arrived at the facility to conduct the Case Management visit in regards to the Exclusion of a staff member, S1. LPA Calandra was greeted by Charlynn Ursua, Administrative Assistant and explained the purpose of the visit.
According to the Administrator, Chidi Ikeme, S1 does not currently work for the facility and has not worked there since 2020.
During the visit, LPA received a copy of the facility's staff schedule for the week showing that S1 is not currently working at the facility.
No deficiencies cited during today's visit.
An exit interview was conducted and a copy of the report provided.
