The inspector’s account

On 3/4/2026, Licensing Program Analyst(LPA) John Calandra arrived at the facility to deliver an Amended report regarding complaint #26-AS-20251027233036. LPA Calandra was greeted by Elizabeth Esquies, House Manager and Lorraine Baca, Co-Administrator and explained the purpose of the visit. Licensee/Administrator, Chidi Ikeme arrived later during the visit.

LPA Calandra explained to the Licensee the reason for the Amended report. Due to technical difficulties, LPA Calandra was not able to attach the deficiency page to the Amended 9099 report. Thus, LPA Calandra attached the deficiency page to this case management report.

Deficiencies are cited under the California Code of Regulations. Failure to correct the deficiencies by the POC due date may result in Civil Penalties.

An exit interview was conducted. A copy of this report along with Appeal Rights were provided.
