On 9/17/2025, Licensing Program Analyst(LPA) John Calandra arrived at the facility to conduct the Annual 1-year required visit.
LPA rang the doorbell and knocked twice but no one answered the door. LPA contacted the Licensee and Administrator and left voicemails requesting a callback.
No exit interview conducted as no one was available to meet. LPA to send a copy of this report to the Licensee.
