Facility Evaluation Report
On May 27, 2025, Licensing Program Analyst (LPA) Kiran Jain arrived at the facility to conduct a Case Management – Other Inspection visit. Upon arrival, the LPA met with the Administrator (ADM), Roba Abushaaban. The LPA disclosed the purpose of the visit.
The purpose of this visit was to hand deliver an immediate exclusion letter for a staff member (S1), who the Department determined engaged in conduct inimical.
The immediate exclusion letter for S1 was handed to the Administrator.
The administrator confirmed S1 was working for the facility, but S1 was currently not present at the facility during the LPA's visit.
The Administrator was informed to remove S1 from any contact with clients and not allow S1 to be physically present in the facility.
LPA advised Administrator to separate S1 from the facility roster and email the updated LIC 500 to the LPA.
No deficiencies were cited during today's visit.
An exit interview was conducted with the Administrator. A copy of this report was discussed and provided to the Administrator, Roba Abushaaban, whose signature on this form confirms receipt of this report.
