Facility Evaluation Report
On 02/18/2026, Licensing Program Analyst (LPA) Jaime Vado conducted an unanncouned case management - incident visit in response to an SOC341 received on 02/02/2026 via fax. LPA Vado met with new administrator Nick Catalano and explained the purpose of today's visit.
During today's visit, LPA interviewed the administrator regarding the incident, in which it was confirmed as occurring. Per report, a staff person (S1) pushed a resident (R1) on 01/29/2026 at around 5:30pm to 6:00pm. This was witnessed by another staff person (S2) and it was reported to the administrator the same day it occurred. There was an internal investigation conducted and the facility confirmed it taking place based on eyewitness interview of S2. In response to the findings the administrator suspended S1 the same day the incident took place. Assessment done on R1 was conducted and there were no injuries found on R1 and no changes to the status of R1.

Report is reviewed with administrator and a copy is provided.
