Facility Evaluation Report
On 1/14/2026, Licensing Program Analysts (LPA) Yi Sam Jian arrived unannounced for the purpose of conducting a case management. The purpose of the case management is to address the corresponding complaint investigation report number 26-AS-20251027221630. LPA met with administrator, Tyler Manzo. LPA explained the purpose of the visit.
Regarding the allegation that staff mismanaged resident medication, the Department conducted an investigation and determined that Resident R1 did not receive the prescribed morning dose of medication due to a depleted medication supply. Although the facility provided documentation indicating that measures were taken to address the depletion of the medication supply, the facility did not arrange, or assist in arranging, for medical care appropriate to the resident’s conditions and needs to prevent the medication from running out. This resulted in a technical violation.
This report was reviewed with the administrator, and a copy was provided to the facility.
