Facility Evaluation Report
On May 27, 2026, Licensing Program Analyst (LPA) Murial Han conducted an unannounced annual inspection. LPA met with Program Director, Rachel Jimenez and LPA explained the purpose of today's visit.
LPA toured the facility with the program director and observed the multipurpose room, and three classrooms (#1, #2, and #3). LPA observed participants were participating in various activities by choice with staff members. The hallways and emergency exits were free of obstructions. The facility observed to be clean and tidy, comfortable temperature is maintained and lighting is sufficient.
LPA observed sharps, toxins and disinfectants were locked and inaccessible to participants in care.
The hot water temperature measured at 108°F.
A review of (6) resident files was conducted and noted on the LIC 858.
A review of (5) staff files was conducted and noted on the LIC 859.
This report is reviewed and discussed with the program director and a copy is provided.
