The inspector’s account

Licensing Program Analyst (LPA) David Marrufo conducted an unannounced Required - 1 Year visit and met with Program Coordinator Sam Leale.

During visit, LPA toured the facility inside and out. LPA toured the kitchen area and observed the food storage areas.

LPA toured the activity room and the outside area. LPA observed the outdoor exits to be clear of obstructions.

LPA reviewed seven resident records and found them to be complete. LPA reviewed four staff records and found them to be complete.

The Annual Fire Alarm Inspection Report indicates the last inspection was conducted and passed on 06/20/2025. The Emergency Disaster Drill Log indicates the last drill was conducted on 10/16/2025.
