Facility Evaluation Report
On 10/24/2025 Licensing Program Analyst (LPA) Maria (Mita) Partoza, conducted an unannounced required 1 year inspection, met with administrator (ADM) Bonnie Jiracek-Codd and stated the purpose of the visit.
The facility serves adult 18-59 years old who are ambulatory and developmentally challenged. The facility has a current census of 4 resident. 2 Out of 4 resident and 6 staff are present at the facility at the time of the visit . 2 Out of 4 resident are out attending day program. The 2 residents present at the facility are both resting.
LPA toured the facility with ADM inside and outside. LPA inspected 4 resident bedroom, 2 bathroom, living room, kitchen, office, garage laundry and backyard area of the facility. LPA observed that the backyard, walkways, hallways and exits are free of debris and from any obstruction.
Bathroom were equipped with grab bars and nonskid mats. Each resident has their personal hygiene supply caddy, labeled and kept in a secure place. The toxins, cleaning and laundry supplies are kept in a locked cabinet. Linens and extra towels are kept in the laundry area. The facility has working washer and dryer. LPA observed that water closet lids are secured for safety, bathroom mirrors are shatter proof. Medications AR kept in a locked cabinet inside the office. Sharp objects were locked and inaccessible to residents. Facility is equipped with a well water reservoir.
