The inspector’s account

Licensing Program Analyst (LPA) Marcela Yanez conducted an unannounced case management visit. LPA met with Emi Nagai, Administrator. LPA announced the purpose of the visit.

During Annual inspection while reviewing staff records LPA found an incident that occurred on 12/16/25 where a client eloped and exited the building. Staff found client at the corner street near the facility. ADM stated that the facility was having a performance event and door was slightly opened and client slipped out.

ADM stated the client was gone approximately less then 10 minutes. ADM stated the staff walked client back to facility. ADM stated the Responsible party was notified and care plan was created. ADM stated that 3 staff were working during incident. LPA reviewed client file, Per client LIC602 states the client is not allowed to leave facility unattended.

The facility staff did not send in a LIC624 to the department. LPA informed ADM the need of reporting incidents that occur with clients.

Deficiencies were cited as per California Code of Regulations Title 22 see LIC809-D. This report was reviewed with Emi Nagai, Administrator and a copy of this report and appeal rights were provided.
