Facility Evaluation Report
Licensing Program Analyst (LPA) Marcela Yanez conducted an unannounced Required 1 Year visit and met with Emi Nagai, Administrator. LPA announced the purpose of the visit.
During visit, LPA toured the facility inside and out. LPA observed a kitchen area and a refrigerator mainly used for staff. LPA observed the day program room where they have tables and activities for clients. LPA observed 4 staff in the day program setting up for clients arrival.
LPA observed a locked cabinet for chemicals. The facility does not have any sharps stored in the day program. LPA toured the outside area and found the exits to be clear of obstructions. LPA observed fire extinguisher was last serviced on 6/16/25. LPA reviewed Fire and Earthquake log. The last earthquake drill was conducted on 10/16/25 and fire drill conducted on 05/23/25. Staff tested the carbon monoxide detector and found the carbon monoxide detector to function properly.
The facility has a fire sprinkler system that was last serviced on 03/18/26 by third party agency.
During tour LPA observed a ceiling tile in the day program to be damaged and had a piece missing. ADM stated there was a leak in the HVAC system over the weekend. ADM stated that the tile will be replaced.
