Facility Evaluation Report

Licensing Program Analyst (LPA) Maria (Mita) Partoza, conducted an unannounced case management visit for the incident that was reported on 10/16/2025. LPA met with Executive Director (ED) Kellie Shearer and stated the purpose of the visit.

During the visit, LPA gathered information such as but not limited to LIC 602 (Physician's report), appraisal needs and services plan, staff training logs was emailed to LPA and LIC 500. LPA conducted an interview with ED and with resident. Based on this information it has been determined that the case management needs further investigation.

No deficiencies were cited during today's visit based on California Code of Regulations (CCR) Title 22. An exit interview was conducted with ED Kellie Shearer and a copy of the report was provided.
