Facility Evaluation Report
On April 30, 2025, Licensing Program Analyst Manuel Monter conducted an unannounced POC visit. LPA met with Lead Kara Monteclaro. LPA explained the purpose of the visit.
On April 17, 2025, the facility was cited the following type A deficiency during a compliant investigation, regarding a resident wandering away from facility staff's field of vision, walking towards a neighbors property. This occurred on April 10, 2025 with the POC due of April 18, 2025. The following code sections were cited on April 17, 2025: 80078 Responsibility for Providing Care and Supervision (a)
On April 18, 2025, the Department received the written plan of corrections.
On April 30, 2025, LPA conducted a POC visit to clear the Type A POC's. A copy of the Letter of Deficiencies Cleared letter was provided.
No deficiencies cited during today's visit. This report was reviewed with Lead Kara Monteclaro and a copy of the signed report was provided.
