Licensing Program Analyst Manuel Monter conducted a POC case management visit to clear deficiencies cited on August 29, 2025 and met with Staff Rita Rabanal. LPA explained the purpose of the visit.

LPA received plan of corrections for type A deficiencies by August 30, 2025. Deficiencies cleared during todays visit. POC cleared letter provided to ADM.

No deficiency was cited during todays visit. This report was reviewed with Administrator Patrick Binaro. ADM stated his staff, Staff Rita Rabanal could sign on his behalf. A copy of this report was provided.
